• Online nomination – Complete and submit a nomination form here.

• Offline nomination – Complete a hard copy by either downloading a nomination form or picking up a form in the school’s front office.

In addition to identifying information, the following materials must be submitted for an individual to be considered. Failure to supply supporting documents will invalidate the nomination and the nomination will not be considered.

• A letter of recommendation from the person making the nomination, and other letters of support from appropriate individuals who can validate or substantiate pertinent information related to the nominee.

• A LIMITED number (up to 10) of newspaper clippings, magazine articles, copies of scorebooks and statistics to help substantiate the accomplishments and contributions of the nominee.

• A suitable photograph reflective of the nominee’s time at Snow Hill High School (for reproduction).
